You don't.: Even with the windows down, it is very serious business.
The elderly have a very difficult time regulating their internal body heat.
Figure out a way to avoid this scenario altogether, or else have the person wait outside of the care in a safe area.

Probably not: My general rule of thumb is ' if you can not get down to the floor and get back up on your own safely, then you should not be doing floor exercises...

A Wheelchair-lift: Beyond the obvious direct assistance to the person slowly getting in-out, up-down the car/suv, something to consider is a power wheelchair lift installed as pictured here. It will cost some money, but a very hepful tool to help those needing it, especially if the person is large and cannot be safely assisted by another person. Best of luck to you.
