Irritation??: This could be an irritation you can see things like this when people put chewing tobacco in one place too long, or hold mints or candy as a habit in the same place for long periods of time.  I would analyze your routines and see if this applies to your situation, i would then have your dentist do a routine oral cancer screening and if necessary he/she will send you to an oral surgeon for biopsy.

The : The roughness and patches in your mouth can be attributed to a couple of things - and infection or a reaction to a type of food you ate.
Foods - some people experience excoriation of oral membranes if eating a food that they are sensitive to. For instance, if you eat a fruit that is highly acidic such as kiwi, it can be irritating to the mouth. Some people experience sensitivity to spicy foods or cinnamon flavored candy/food. This is resolved by avoiding foods that can cause irritation.
Infection - there are fungi that reside in our bodies that may grow out of control if our immune system is affected or decreased. One infection that can occur in the mouth is called "thrush, " caused by the fungus, candida. This infection is manifested by red or white, irritated patches in the mouth. It can be treated with otc topical solutions, but please note that healing could take a couple of weeks. It will be important to follow up with your doctor if symptoms persist, despite otc treatment. Oral cancers and other infections have similar early symtpoms, as you are currently experiencing.

It is a result of the microscopic elevations and depressions of the surface of the gingival tissue due to the connective tissue projections within the tissue. You can confirm this with your own dentist.

Sudden? Always?: At age 16, you may not have a crowned (= "capped") molar, but if it is covered with a porcelain crown, the porcelain may have chipped/peeled. If so, depending on the condition you may have your dentist polish the surface, or you may need to have the entire crown replaced--it depends.  If it is your own tooth with a similar incident, you will need treatment. Need to see a dentist to find out.

It is usually harmless and not contagious. Seek medical attention, it could be eczema, psoriasis, or skin cancer.
