Dr. Scott Bolhack answered:
Could medical leech therapy help someone with infected leg ulcers (necrobiosis lapoidica)?
Leeches leg ulcers Stringio
Leeches can be effective when an area has venous congestion; the leeches withdraw the blood from the congested area. Medicinal leeches are different than those obtained elsewhere. There are no studies of value showing that they help with infection.

Dr. Scott Bolhack answered:
What can I do about Pressure sore on my heel?
Heel sore Sores
A pressure injury is due to pressure. The first step in healing is to keep pressure off of the heel. A clinician will help you analyze why there is pressure on the heel and suggest ways to offload that pressure using cushions, pillows, special foot gear such as pressure relieving foot ankle orthotics (PRAFO).

Dr. Scott Bolhack answered:
Do pressure sores require surgery?
Pressure sore Pressure_sores
Most pressure sores, once identified and treatment initiated, do not require surgery to treat. Surgery is reserved for severe and or deep injuries in which the best course of treatment is to have surgery performed to close before other issues arise (infection, debility, etc.) These surgeries are complicated and should be performed by a surgeon that does many of these procedures.

What is a pressure sore?
Cause Pressure Sore Stringio
Pressure causes a bed sore. One unknown is how long does it take a pressure ulcer to develop. There are many risk factors including diabetes mellitus, low blood pressure, moisture from incontinence or perspiration, fungal infection, use of medical devices, etc. We simply do not know for an individual patient whether rotating them every five minutes or every two hours will prevent pressure injury.

What are ways to prevent pressure sores?
Bedsores Open-uri20140403-29561-cozyfo
Bedsores are due to pressure over bony prominences. The best way to prevent bedsores, once they occur, is to keep pressure of these areas. That means that you need to constantly move the person into different positions to relieve the pressure. Seeking professional help like with a wound center is a great option since depending on the depth, stage and location of the uler, your options will differ.

How can people on wheelchairs prevent pressure sores?
Sores from sitting Stringio
Sores from sitting in wheelchairs are due to all three types of pressure injuries: pressure, shearing and friction. Pressure can be relieved by alternating positioning; shearing can be prevented by careful attention to lifting oneself before moving into a new position; and friction can be prevented by understanding and correcting any rubbing issues.
