Not usually: HPV does not usually cause urethral pain or urethritis. If you're having pain, it is important to get tested for other STDs such as gonorrhea and chlamydia, and even HIV and syphilis if you have had unprotected sex recently. The first two are urine tests, the second two are blood tests. These can be done at your doctors office or urgent care.

Dermatologist: I would see a dermatologist. There are many treatments for acne, but for more severe cystic acne, a specialist is needed since the treatment is more involved and there are side effects to the medications that need to be discussed.

Probably not: Ear wax does not cause pain and there is no need to clean it out. Infants often grab their ears if they have pain, either from teething or an infection. If he is drooling, he may be teething. If he has cold symptoms, it may be an ear infection. If it really seems to bother him, try Tylenol for pain relief. If it persists for many days or he seems ill with fever, he may need to be seen by a doctor.
