I am Board certified in Anesthesiology and Pain Medicine and have been practicing for nearly 30 years- currently located on the Monterey Peninsula on the Central Coast of California. I practice in a multi-disciplinary musculo-skeletal group of physicians specializing in orthopedics, neurosurgery, spine surgery, total joint and hand surgery, physiatry, and podiatry. We are the largest and most comprehensive group of musculo-skeletal treating doctors in the area providing support in the diagnosis and treatment of pain. I am particularly interested in spinal interventional procedures such as epidural, facet, radio frequency neuroablation, peripheral and spinal cord stimulation. The procedures are performed under X-ray guidance (fluoroscopy) in an out-patient surgical suite; ultrasound guided injections are also done in the office. My goal is to maximize function through pain management techniques without a total reliance on opiate medications. Opiates should be used judiciously and other avenues with non-narcotic medications, physical therapy, cognitive behavior therapy and home exercise should be considered and explored. Most importantly an accurate diagnosis must be made with a comprehensive history, physical exam, diagnostic studies including advanced imaging before beginning effective treatment-appropriate management of chronic pain is the key to function and improved quality of life.

Conservatively: With musculo-skeletal pain treat with pt and home exercise. Medications for nerve pain can be used but cause side effects. Consider topical compounding ointment, followed by non-opiate meds. Anti-seizure meds like neuron tin, tricyclic antidepressants, snri antidepressants like Cymbalta (duloxetine) have been effective. Nerve blocks, neuroablation, spinal cord or peripheral nerve stimulators can be effective.

Discogenic pain: Following spinal fusion the adjacent disc above or below the fused site can bear the weight load causing back pain or recurrence of leg pain. Check the placement of the spinal cord stimulator with an xray to determine if the leads have moved if pain relief has stopped. A ct scan may be indicated to determine if there is re-herniation, spinal stenosis, spinal cord or nerve root compression.

Conservative care: Pt should be initiated progressing to an independent home exercise program performed daily. It is not uncommon to find small tears in the rotator cuff, which may not require surgery. Other alternatives include applying compounding topical medications to the affected site for pain control. Acupuncture is another option effective for musculo-skeletal pain. Lastly surgery is the last option.
