Maybe: A typical workup for this would include an MRI of the brain and possibly a ct scan of the ear bones. A full workup (history and physical) is necessary to look for additional signs and symptoms that would lead to the diagnosis. Only after an accurate diagnosis is made can treatment options be discussed.

Nasal congestion: Adenoids sit behind the nose. In children enlarged adenoids are somewhat common and can result in nasal congestion, post nasal drip, runny nose, and contribute to sinus infections. Usually this is short-lived and no therapy is necessary but if lasting more than a month, consideration of nasal steroids and saline is made for treatment. Sometimes an adenoidectomy is performed.

Not necessarily: Enlarged tonsils and adenoids in children is a common finding. Unless the tonsils and adenoids are causing more problems than they are worth (labored breathing at night is impacting the quality of sleep or there are recurrent infections) then the size is of no great consequence. A visit to an ENT doc is still recommended to fully evaluate.
