Go to dentist: The only advice in this situation is to go see a dentist urgently and they may refer you too the oral surgeon or better still go see the oral surgeon if the tooth was extracted already

Fracrured porcelian: You have possiblly fractured the porcelain of the crown and may need to see a dentist to either bar the crown re done or some sort or repair although re sticking porcelain back on isn't possible so he or she may reccomend a new crown.

Absolutly: It is very possible that your orthodontist may recommend extractions this occurs when you h e more tooth structure too the size of the jaw then two or four teeth usually need to be removed although not the first option but sometimes becomes the only option.

Talk to orthodontist: You really should talk to your orthodontist and ask him u probably have very inflamed gums due to improper oral hygiene

Yes: If the gums are necrotic or the gum disease is very prevelant you may need surgery to help improve the situation it is called as periodontal surgery and it's like plastic surgery for the gums.

Cleaning Required: There is always bleeding from gums if your not visiting the dentist every six months. The reason is possibly due to inflammation of the gums due to deposits and these need to usually be removed professionally by a dentist or hygienist.
