Doctor Q&A for Dr. J David Holcomb

Yes, lower facelift: Lower face and neck lift will not worsen dry eyes. In fact successful mid face lifting can have a slightly beneficial effect on lower eyelid position. Brow lift and upper eyelid lift procedures, however, can create a temporary (rarely permanent) worsening of dry eyes. If your eyes are dry consider having an evaluation by an eye doctor before scheduling upper eyelid lift or brow lift surgery.

Several months +: Although initial healing and resolution of most of the swelling and bruising takes place in the first 2 weeks it can take several months (6 weeks +) for the nasal bones to heal. So it is important to avoid a significant bump to the nose during this time so that the healing nasal bones are not moved apart again...

Not always: So called "breaking" the nose during rhinoplasty (nose job) is really a very controlled cut or separation of the nasal bones (called an osteotomy) so that they can be moved inward to narrow the nose. Rhinoplasty does not always require narrowing of the nose. This depends on the pre-existing shape of the nose and the reshaping goals that you and your surgeon agree upon before surgery.

Many candidates: Laser skin resurfacing now encompasses traditional full field (complete skin coverage), fractional (partial skin coverage that is ablative vs non-ablative, ie removal and intense heating vs milder heating of tissue) and combination (hybrid) techniques. These different approaches enable treatment of light, medium and darker skin types and many conditions (eg acne scarring, sun damage, wrinkles).

Possibly: It depends on how dry your eyes are and whether there are any related changes on your cornea that only an eye doctor with special equipment can observe and document. You may be able to have an upper and/or lower blepharoplasty but check with your eye doctor first. If you proceed your eyes may become more dry (at least temporarily) and you may need to use eye drops more often and/or add eye gel.

Most likely: If you put your finger on the top of the nose about half way between the eyes and the tip you are feeling the upper part of the nasal septum. Septoplasty surgery usually involves working on the middle and/or lower part of the septum (far away from the part of the nose you are touching). The goal is to open up the airway without disturbing the structural support or changing the shape of the nose.

Several options : A thin face may be related to underlying bone structure, atrophy (gradual loss) of soft tissue (eg fatty tissue, muscle) and the aging process where sagging tissues create a thinner, longer face. Soft tissue augmentation (structural fat grafting; cheek implants; filler injections) and/or lifting sagging tissues (face/neck lift) can effectively improve facial volume and overall appearance.

Yes: Men, women, boys and girls (generally age 14 and up) commonly undergo rhinoplasty (nose job) and chin augmentation during the same operative session. A short chin tends to create the perception that even a normal sized nose is large. So adding length to the chin helps to bring harmony and balance to the face.

Not the same: "microabrasion" actually refers to a dental procedure to treat the outer enamel of the teeth. Microdermabrasion is a skin procedure that uses manual exfoliation to remove the outer layers of dead skin to improve skin texture and penetration of topical products. Microhydrabrasion is a skin procedure that uses suction, a liquid vortex and mechanical tip to remove the outer layers of dead skin.

Very safe: But it is surgery that involves some form of anesthesia. Find a board certified (eg abfprs or abps) facial plastic surgeon with current hospital privileges and make sure that the surgery will be performed in an accredited facility (eg ahca for most surgery centers; aaahc or aaaasf for most office-based facilities) and that you are comfortable with the proposed anesthesia method and provider.
