Likely not shingles: While it is possible for a person to get shingles more than once, it is quite rare, especially in the wake of the first attack. Your antibody levels to herpes zoster (the virus that causes shingles) are high and should prevent another infection for at least a while. More likely some there is some other cause behind the pain.

Your bmi is 28.8: Bmi stands for body mass index. A reading of 20-25 is normal; 25-30 is overweight; and 30 and above is obese. So yes, you are overweight. If you get down to 160 lbs.(easier said than done) your bmi would be 25.8--much better.

A cosmetic problem: Toenail fungus is a fairly common condition, more so as a person ages. It rarely leads to any significant medical complications. For that reason insurance companies do not cover the cost of treatment. There are 2 medications that work about 50% of the time. Laser treatments have had similar success. If left untreated the nail thickens and discolors.
