Yes you must: There could be capsular changes that can be easily managed to improve your vision. Retinal health and pressures should be reviewed too

No: Refractive errors (farsightedness or nearsightedness or astigmatism) usually have caused in the shape of the cornea, axial lenght of the eye or lens based changes. None of these elements are medically influenced presently and hence you cannot treat refractive errors without some form of corneal/lenticular modification ie. Refractive surgery

Both: Depends on your goals prior to lasik surgery as your presbyopia can be accounted for with planned blend vision/monovision/presbylasik preoperative simulation sometimes avoids such disappointment. This can still be corrected. Ask your surgeon.
