At birth,not heredit: The former is a congenital but not hereditary disease involving usually one kidney forming a mass of cysts without functioning kidney tissue. Found with prenatal ultrasound. Kidney shrinks and the other kidney assumes its work. The most common form of PKD which is hereditary results in increasing cyst number and size over many years. Other organs have cysts and kidneys fail over many years.

Aquired versus genet: Multicystic kidney dysplasia is a congenital abnormality of the connective tissue usually of one kidney which is a mass of cysts and fibrous tissue. That kidney has little or no function and can be detected with pre natal ultrasound. The disorder is not genetic. Polycystic kidney disease is a genetic disorder resulting in gradual cyst growth in both kidneys with gradual loss of kidney function.

Probably not: We would need to take a history to assess other medications in use as well as perform a physical exam to assess the state of hydration. The high bun/creat ratio and the slightly high Albumin suggest volume depletion. A urinalysis would be helpful in screening this patient for renal disease. The patient's current weight would also be needed. Cystatin c testing and a GFR nuclear scan could help.
